clm3loresMid-America Transplant Services & Heartland Lions Eye Banks invite you and your family to an interfaith ceremony honoring the Organ, Tissue and Eye Donors who have passed on the Gift of Life.

Join us at 10:00 a.m. to decorate a special candle to be included in the ceremony.
The ceremony begins at 10:30 a.m. at Campbell United Methodist Church (1747 East Republic Road, Springfield, MO 65804)

Please RSVP by Tuesday, April 16, 2013 by calling 314-735-8218 or 800-925-3666. You will be connected to a voice mailbox. Please leave the following information: (1) Donor’s name; (2) indicate if you are a donor family, recipient, or friend; (3) # of people attending; & (4) your name and telephone number.

We know that this may be a difficult time and you may not decide to attend until the morning of the ceremony. Please feel welcome to attend even if you did not RSVP.

Honor Your Loved One in a Special Photo Presentation
If you will be attending the ceremony, we invite you to submit a photo of your loved one to be part of a special photo presentation. Please send the photo via email to donorfamily@mts-stl.org in jpeg format. When submitting your photo, include the name as you would like it to appear.  If you have submitted a photo previously, please notify us of the year the photo was submitted.  We will confirm with you that the photo will be included in the 2013 photo presentation. If you have any questions, or would like to mail your photo, please call 1-800-925-3666.
